Incident Details

September 1, 2013 Location of the incident: , Type of Incident:

I am writing this in hopes that others may learn from my very costly mistake. I took off on a beautiful evening. The launch was perfect and everything felt good. At about 100 feet I looked up at the wing to inspect the wing and lines. As soon as I arched my head back, my helmet came off my head and went right into the prop. The helmet exploded into shrapnel and the prop was destroyed. This accident occurred because I had forgotten to buckle my chin strap. This was a $500.00 communications helmet and a $450.00 prop. I have now added checking the chinstrap to my written checklist that I use for most items on the preflight. This could have been a very serious accident. I encourage all PPG pilots to use a written checklist and stay focused during your preflight.
