Pilot was on trying a machine that was not his. Other people set it up, hooking in the risers. The main attachment straps that hold the carabiner were connected in the wrong place, below the intended metal loops. That made it possible for the straps and connected risers to slide foreward and backwards which was not obvious at first. The inflation and launch went well but, soon after liftoff, the motor slid backwards which caused it to tilt severely downward. That induced a powerful twisting force and the pilot did a nearly complete turn-around before reaching 10 feet with the motor pointing nearly straight down. Lacking any thrust, he came down and hit, cage first, mostly on his back.
